Survey for five-year service delivery plan

For the past year, the Martin County Healthy Start Coalition has been developing its five-year service delivery plan.

So that we may gather community feedback, we are asking our clients to whom we have provided services, for their help by responding to the following survey. This survey is for pregnant women and women who are postpartum up to 24 months.

This will help us complete our needs assessment portion of the service delivery plan before we dive into action planning this spring.
